Documents and FormsTo apply for the Federal Work Study program, please print, complete and return to the Financial Aid Office. To apply, you will need to submit your FAFSA and Financial Aid Application also.
The PTS Seminary encourages students to register to vote. If you are not registered to vote, please visit the website for National Mail Voter Registration Form. Please submit this form and any questions you have to your state's designated voter office. Do not mail this form to PTS.

Educational documents are protected under the Family Educational rights and Privacy Act (FERPA). Therefore, students that wish to release financial aid information to another party (spouse, parent, etc.) must print and complete the Financial Aid Release form and return to the Financial Aid Office or Business Office for your file.

If your application was selected for review in a process called, "Verification." Your school will be comparing information from your application with signed copies of your 2010 Federal tax forms, W-2 forms, or other financial documents. The law says we have the right to ask you for this information before awarding Federal aid. If there are differences between your application information and your financial documents, you or your school may need to make corrections electronically or by using your FAFSA.

To complete your registration process and receive your financial aid awards, the Attendance Verification Form must be completed. Once your professors have signed off on your attendance sheet, please return this document to the Business Office.
